Consent form for research participants

Thank you very much for agreeing to take part in a focus group for a project called Developing a Regional Childcare Strategy for the North East Combined Authority (NECA). 

We would like to remind you that participation in the study is entirely voluntary and you can change your mind about taking part at any time before or during the focus group.  

– If you change your mind before the day of the focus group, please email Alicia.Knights-Toomer@coramfamilyandchildcare.org.uk to let us know you no longer wish to take part. 

– If you change your mind during the focus group, you can leave the meeting and let us know if we can or cannot use the information you have provided up to that point.
